In this paper, I’m going to outline some of the thought processes, decisions and issues we had to face when evolving a Microsoft .NET application from 1.x/2.x to 3.x. I’ll look at how we helped our client to adopt the new technology, and yet still maintained a release schedule acceptable to the business.

I will avoid details of the proprietary aspects of the system, and I’m not providing any source code for the components we’ve built. On the other hand, I will cover in some depth the general architecture and design of the system, particularly as it relates to .NET3.x. As a bonus, there are quite a few code snippets and tips we picked up along the way.

The purpose of this paper is to review the current status of preventable blindness in Ireland. Based on the growing epidemic of diabetes and its associated ophthalmic complications, together with the recent increase in both glaucoma and age-related macular degeneration, we estimate that there are now in excess of 200,000 people at risk of preventable blindness in Ireland.

In this paper we review the current situation and discuss a technology solution similar to that deployed and in use in several other countries. We review the technology in the context of the Kiely report and its recommendations in respect of Retinal Screening.

Clinical ophthalmic services have seen a significant increase in demand over recent years, unfortunately staff numbers and budgets have not increased proportionally. Now with the introduction of systemmatic community based blindness prevention programs such as those for Diabetic Retinopathy, Macular Degeneration, Glaucoma and Cataracts even more patients will require assessment and specialist treatment. Add to this the introduction of new anti-VEGF compounds, the addition of new imaging technologies such as coherence tomography, and improvements in retinal surgery techniques and it is clear that demand will easily outstrip supply and patients will suffer.
